Where these applications come unstuck
The narrow named field is the main trap: a general health-administration, medical-records or hospital-management background without a qualification titled specifically in health information or medical records management risks a mismatch, even where the day-to-day duties overlap heavily with what this code assesses. The exclusion of Records Manager cuts in a specific direction too — general records-management experience, however senior, is explicitly not treated as equivalent evidence here, and vice versa on Records Manager's own page.
